This website does not collect, store, process, or transmit any Protected Health Information (PHI). Our online forms are intentionally limited to basic contact and scheduling information only. All clinical discussions, including diagnoses, medications, treatment plans, and medical history, take place privately by phone with our licensed coordinators. This is a deliberate design decision to protect your privacy.

Protected Health Information (PHI) is any information that can identify you and relates to your health, medical condition, healthcare treatment, or payment for healthcare services. This includes things like diagnoses, medications, medical history, and health insurance information.
