For caregiver applicants, we also collect certification type, years of experience, availability, areas of specialty, and the boroughs you can work in.

What We Never Collect

We explicitly do not collect, store, or request any of the following through this website: diagnoses, medical conditions, medications, treatment history, insurance information, Social Security numbers, financial account details, or any other protected health information (PHI) as defined under HIPAA. Our forms are intentionally designed to prevent this. If you attempt to include this information in a form field, please be aware that it is not reviewed or retained by our coordinators, all clinical discussions happen by phone in a secure environment.
